|So this is about the backstory of Miyu, told from the perspective of the Shirou from the alternate world who became Miyu's brother.|
5 years ago, when Kiritsugu was still alive, Fuyuki City was caught in a big explosion. Shirou went off to try and save some people, and found a little girl who turns out to be a "Child of God". These children have god-like powers and are kept hidden away until they turn 6, and Kiritsugu wants to try and use this girl as a tool to save the world. This girl, named Miyu, starts to like Shirou a lot... Kiritsugu dies before Miyu turns 6, and now Shirou has to try and figure out how to use her powers to save the world...
Shirou is in high school and is still looking after Miyu, and is trying to teach Miyu about the world so that he can take her outside and let her have a proper life... Sakura often talks to him after school but doesn't go to his house or anything... Also he eats lunch with the student council president Julian every day... Apparently at this point Fuyuki City is still somewhat populated and not completely abandoned yet.
There was this really cute scene where Shirou and Miyu are looking up at the stars, in the same place that Shirou and Kiritsugu used to, and Miyu wishes that she can become Shirou's real sister... from then on she starts referring to Shirou as onii-chan.
When Shirou goes to the elementary school thinking about taking Miyu there, the little blonde girl (Erika) spies on him suspiciously... Later, when Shirou takes Miyu back to her family's grave, at the place where he originally found her in, Julian and Erika arrive and reveal their intentions. Julian was the one who caused that big explosion years ago, and he noticed the power of a Child of God from this place during that explosion, and has since been keeping watch on this place hoping for the child's kidnapper to show up back here again...
Shirou tries to fight Julian and gets pummeled by Angelica. Julian takes Miyu, and then Shirou later wakes up to the priest Kirei talking to him...
I am a priest that is responsible for witnessing the end of this world. Welcome, lost lamb.Shirou is surprised that he already knows about Miyu and everything... he tells Shirou more about the Ainsworths, and how they're trying to use Miyu to save the world, just like what Kiritsugu wanted to do.
Julian, the last surviving member of the Ainsworth family, is trying to start another Holy Grail ritual using Miyu as the vessel... So I guess in this timeline, the Ainsworths are one of the main families responsible for the Holy Grail War.
Shirou is now spending every day trying to break into Julian's stronghold and save Miyu... he invites Sakura over for dinner, and as she leaves, she gives him her servant card, which contains the King of Heroes Gilgamesh, and asks him to take her place in the Holy Grail War so he can save Miyu... but then she cries and asks him to run away from the city instead and protect himself.... Suddenly, Shirou and Sakura get attacked by Sakura's brother, who also has a servant card, Assassin. With that card installed, he's even more of a murderous lunatic than before...
Sakura tries to install Archer and fight her brother, thinking she'll win easily, but the card doesn't work at all... Assassin kills her and then goes after Shirou... Here's where Shirou finally starts to become more badass.

He installs the Archer card and beats the shit out of Assassin... Just when he's about to finish him off, he sees Sakura's scarf fly away, and he realizes the Assassin he's fighting is a decoy, and kills Sakura's brother who is just about to attack from behind.
Sakura's brother suddenly gains his memories back, and realizes he's been had by the Holy Grail War's creator... oh, so that wasn't Sakura's brother at all, it was a doll made by Julian... that's what she meant when she said her brother has been dead for a long time.
Shirou is now going around fighting all of the other card-wielding masters, who all turn out to be fake doll people set up by Julian. His personality now is very similar to the Heroic Spirit Emiya.
After losing everything, my goals became clear.
He defeats all 7 other masters and collects their cards, and then Kirei tells him where he can find Miyu.
When he confronts Julian, he talks about how Julian is following the path that he himself was supposed to follow... but now Shirou is diverging from Kiritsugu's path in order to save Miyu. He tells Julian that he thinks the family that created the Child of God would've used that power to wish for their child to grow up living a healthy and prosperous life...
If that's evil... then I'll be evil.
Shirou goes to Miyu and wishes on the Holy Grail for her to have a happy life... I think this is what causes her to get transported into the other world...
Angelica fights Shirou with the real Gilgamesh card installed, the one that Sakura thought she had... Even though Shirou's Archer card was a fake, he was able to summon Heroic Spirit Emiya within himself, and now can fight Gilgamesh on even ground without even installing that card...
Shirou uses his reality marble, and it looks like he's about to win... but then Julius helps out with his Spacial Displacement powers. Gilgamesh defeats him with the Enuma Elish sword which is the ones sword that he can't counter even with all of his unlimited swords combined.
Shirou lost to the Ainsworths, but he declares that he already won... then it shows the beginning of the Illya series from Miyu's perspective. Illya shows up for like 2 seconds in the whole movie.
